Prescription Refill Requests

Looking to refill your Ziering Medical prescribed oral and topical medications to manage your hair loss? To make your experience more convenient, we’ve included direct links to our pharmacy partner, ZVI Health, so you can complete the necessary forms without any hassle.

Upon completion of the Prescription Refill Request Form, ZVIHealth will thoroughly review your request and initiate the refill process. Anticipate delivery within 2-3 business days from the time of submission.

For patients who haven’t consulted their Ziering Medical provider within the past year, it is imperative to fill out both the Patient Portal Information Document and the Medical Refill Request Form.

These health history and medication refill forms are intended solely for current Ziering Medical patients. If you are not currently under the care of a Ziering Medical Physician, we kindly request that you contact our office to set up an appointment before filling out any forms. This paperwork contains patient-specific information and is designed to facilitate efficient communication between our medical team and our existing patients. Ziering Medical requires the submission of both forms for all medication refill processing.
